If you are an employer and want to report unemployment insurance fraud or potential fraud, you can submit a Notice of Claim Filed (Form 220). If you did not receive a Notice of Claim Filed, you can find it on SIDES (State Information Data Exchange System), if you have registered.
If you received a Notice of Claim Filed (Form 220) and your employee says they did not file a claim, contact us immediately at 503-947-1995.
If you did not receive a notice and believe someone is committing unemployment fraud, complete this form or call 503-947-1995 or 877-668-3204 to let us know immediately. If you need to report a job refusal or an employee who has not returned to work when recalled, complete this form.
